Jesus. Quite. The only transsexual I've ever known was gorgeous and blokes would go mad over her, all this 'it' stuff is just pathetic to be honest.

Jesus. You do realise that you are now just as guaranteed to moan about the standard PH answers, as the answer themselves these days, you are becoming your own PH idiom  As for the OP whatever you do, do not do what I did and ask when " the work" will be finished....turns out it was  ETA to add missed "not", whoops

You use female pronouns (she, her etc.) and her correct (current) name. NB there's no such concept in English law as a 'legal' name, whichever name you use is your legal name.
Refusing to use the appropriate name or pronouns is, legally, harassment, as is gratuitously 'outing' her. Data Protection Act covers making sure that records are updated appropriately for her new name, too.
She'll probably be vastly happier and healthier than when you last met - bear in mind that what she's gone through is a) harrowing and b) a very, very positive thing for her, likely lifesaving.

In the minutes if her name was Dave Smith and is now Davina Smith then that sorts that out, just use DS in the minutes if her first name is different for example Brenda then just put DS/BS in the minutes.

it makes no difference as to who is chairing the meeting you simply refer to that person as the chair.

Is that Only Fools and Horses?  But yeah, in respect specifically to transfolk, gratuitously throwing around their former name etc can really get you in s  t. Most transitioning people would take some time off work to make the changeover a bit easier, serious respect to this woman for just getting on with it - can't imagine how hard that must be. (and yes of course she won't look much different than she did before, she can't have had any medical care yet - hormones and so on - that stuff takes ages, even if the person is lucky enough to be able to go private)
