DoYouSpain are good, but will constantly hound you with emails for years afterwards.
We've used MalagaCar before as they have an interesting selection of vehicles (we've had a Berlingo, a Medium Cooper and a New Beetle off them in the past) but they do have some dodgy practices like making you pay for a full tank of fuel up front etc.
You can opt out. I also found them very good when I didn't want a car hire firm they sent me and they just sent me a different one. We've used MalagaCar before as they have an interesting selection of vehicles (we've had a Berlingo, a Medium Cooper and a New Beetle off them in the past) but they do have some dodgy practices like making you pay for a full tank of fuel up front etc.

We've had some pretty good cars from them too, apart from one which we only realised the next day after collection had a big dent in it. We expected them to try and say we did it but when we called them they said, 'This is Spain, all cars are dented' and then brought us out another car which was better than the first one for no extra charge. The petrol thing has never been a problem, their prices have been about the same as forecourt ones.
I used DoYouSpain to rent a small car for 5 days in Mallorca. I paid rental upfront which cost £79 bur was sent to the agent who insisted on the full/empty policy plus additional insurance bringing my total on the day to over 140 euros. The agent told me that they always have people going mad and they hate dealing with them.

DoYouSpain is just an agency like most of the cheap suppliers. Just refuse the additional cover. I always refuse it and they always put a deposit on your credit card. This used to be something like 200 euros but it has gone up and up and can be 600 Euros and it's all designed to scare you into taking the extra cover. I've never had anything taken from the deposit I've left but always take photos of the car when I pick it up.
The hire cost never includes fuel so you will always pay for a full tank of fuel in addition when you collect. They bank on you leaving some in the car, that is why it's cheap - let's face it are you going to err on the side of caution or risk running out and missing your flight?
On the plus side, nobody gets anything for nothing. Have a look on a car hire site comparing the full/empty hirers with the full/full hirers and they are about 40 Euros more expensive. Personaly, I'm happy to leave some fuel in and pay half the going rate. If they didn't have this policy, everywhere would be expensive.
