You really need to ask the relevant organiser because it's likely to vary.

Yes - ask the organiser. I was told I couldn't take my 11 year daughter because (a) the seatbelt would sit too high on her; (b) she wouldn't be able to reach the bulkhead with he feet to brace herself; (c) getting a helmet that fitted her head was unlikely. Some circuits have a minimum age for the pit lane as well.

16 years is general minimum at UK circuits, some TDOs have a higher limit though, wise to check first. I was caught out by this when I offered a ride to a friend 16 year old lad for his 16th birthday.

Or go to Nurburgring on a tourist day, no lower limit. I took my lad when he was 10

I think most are 16 as stated above but i did notice at Llandow they had a minimum age of 12...

The only time I've seen under 16s allowed in car on a trackday has been the charity days at RAF Valley.
