First three panes show the guy dreaming. Final pane shows the guy reaching up to feel the boobies.......

He's in a coma and dreaming during the first three pictures, the fourth one is his family looking on whilst his dreams arouse him, had to chuckle at the kids eyes being covered

He says "are those real".
She says "nope".

They're not real because he's in a coma imagining everything. It's a play on the word 'real', because 'fake' boobs aren't fake, they're real boobs with a bag of silicone stuffed in there somewhere.

And of course the C&H dark twist added to that final pane.

Even without the reference to 'REAL' it still works because for the first 3 frames we assume he's really talking to the girl and then in frame 4 we realise he's in a coma having a dirty dream which is funny in it's own right.
