I use the one provided as part of the BT Broadband service, through the BT Home Hub. Works just fine.

And it's £25 a month.

Do any providers offer it outside of their broadband packages?

I get the Orange Livebox with 8meg BB for £5 a month because I have a mobile with them that is £30 a month. Livebox doesn't have a great reputation but mine has never given a problem and the VOIP is pretty good. You just plug any phone into the livebox and you are away - you can choose a local number for the voip line or an 0845 and manage it online. Calls to landlines in this country or in most others are free. It's quite good really!
