Advice - AST Agreement / Taking back possession

Just after a bit of advice
I have a tenant, that is within their 6 month Assured Shorthold Tenancy period. A fraction over 2 months left.

I wish to take back possession of the property.

Is the correct document to fill in and serve as the one below?

No use Form No. 6A as per link:

Tenant must have received copy of Energy Performance Certificate and Gas Safety Certificate, so to avoid arguments, attached copies of the same when serving Notice. Get your dates right on the form, proof of service, and no earlies than the fourth anniversary date of the original start of the tenancy date.
