Generally this means there's something catching on/in the cable. Because the inner rotates, a sharp bend or kink can cause this, as can muck and rust. It can also be a loose strand in the cable which catches.
You can pull out the inner cable and wipe with clean rag and inspect. DON'T grease the inner, it collects more crap, and the grease can harden. Just make it clean, and perhaps a tiny dab of oil or WD40, but use very sparingly.
When putting back you can generally feel when it engages in gbox end.
It can also be the speedo itself, but they tend just read low (or not at all)
