Car hire in Italy

A group of us (6) are flying to Milan (Malpensa) in early September for the Monza GP but staying around Lake Como for the week. Planning to hire a car and wondered if anyone has recommendations for who to use?
We were in Puglia in June and the hire car company wanted an additional (substantial) fee for insurance (vehicle + passengers)...we ended up paying a smaller fee for vehicle only. Have others got advice about what's the best thing re. Insurance?
