In 2009 Emirates 407 had a tail strike at Melbourne because they put the wrong weight in the computer. It then worked out the wrong take off thrust, and wasn't going fast enough to lift off.
He was 170 m off the runway, and took out a load of landing lights etc before he got airborne.
