Yes some manufacturers offer a bespoke service such as Merlyn http://www.merlynshowering.com/bespoke/(needs to fit a sloping ceiling)
General rule of thumb is basic price + 50% . I had a few made for loft conversions, for fixed screens I just made a template and my local glazing merchant made me a toughened glass panel to suit, much cheaper but only for walk in showers.
