Dormer roof sloping back towards the pitched roof would be my guess...

There should be cavity test to about the patch above the window, but if there's not then it could been running down the cavity from the end of the dormer roof.

Yes, that combined with a poor flashing detail where the dormer roof meets the pitched roof.

Looks like the flashing that you can see wasn't too good a detail.

It's quite exposed. Could even be water sitting on the flat roof being blown back and under the tiles.

There not much tiling above the flat roof. I'd take it all off up to ridge and get a proper look.

Now that I've seen that photo, I wouldn't rule out the power of the wind...

The membrane/flashing/tiles detail and the cavity/insulation/lintol/cavity tray SHOULD prevent the ingress of water. We're saying it looks like it may not be done well in your build. When you look at how close together things are (eg the window is near to the pitched roof line so the cavity tray may have had to be modified - cut or bent - for it to fit or was it just missed out??) that whole area would require some carefull detailing thought to make everything fit and be waterproof.
