Bathroom advice - paint and ceiling

Over the past few months, I've noticed the paint above the shower has started to bubble and in some instances, peeling away. Can anyone please give me any advice in sorting this?

Also, any advice on descaling the shower head will be good too.

I had a similar problem so I rubbed it down, a coat of pva and then a couple of costs of bathroom paint.

I would also consider if the fan is working correctly to remove the steam
