I don't think the flashing is a problem adjacent to the fall pipe, there should be a soaker under the tile to pick up the water, however the apron sat on the tiles under the cill looks a bit short, you want a minimum 6'' sat on the roof ideally; also the cast stone cill doesn't seem to overhang the lead where it turns into the wall, it should and also have a drip on the underside so water is thrown clear and doesn't try to track in behind the lead. Having said that, as was pointed out earlier you really need some one there to have a poke about.

Can you post a photo of the gutters along the eaves please?
I suspect they are full of crap

Have to agree, the flashing should be over the tiles. If rain is getting under the edges of those tiles and there's a small hole in the roof felt or where it abuts the wall, water will find it's way in.

Indeed, although it is correct. It should have a welt on ot though. Simplest fix there is to twist the shoe away from the area, reducing the load in heavy rain.

OK, update from today. I managed to get up fairly close (about a sweeping brush handle away) and:

Cleaned out the crap from the bottom of the shoe by both prodding with the sweeping brush handle and spraying it loose with the hose pipe.
Turned the shoe so it faces away from the wall with the brush handle.
Cleaned out the assorted crap from the guttering that had accumulated (it was blocked).

Whilst up there, I spotted what I reckon the problem is - the flashing has come away from the wall under the window to the right as your looking on the previous pictures. It wasn't noticeable from the roadside or from the drive and we hadn't actually looked out of that window.







So, thanks to everybody who made suggestions (clean the pipe, clean the guttering, check all around as the leak may not be where you think, turn the shoe etc,) - now just need to get somebody to re attach the flashing and I reckon we're back to being dry as a bone! Will also make sure cleaning out the gutters goes on the list of regular household maintenance jobs to do.

Hopefully this is more of a handyman Job rather than a specialist roofer so fingers crossed not too damaging on the wallet.

Few hall clips in there to wedge it, and some leadmate. Sorted.
