I know this suggestion doesn't meet your needs but it may give you inspiration. I have just come back from a week in Miami. Yes it is too far for you but the concept of where we stayed was perfect. We rented a private apartment in a 5 star hotel (Setai). It's common to have private residences with all the hotel amenities. This gave us 1200 square feet of living space with 2 beds and 2 bathrooms. We could put the kids to bed and sit on the balcony eating delivered food drinking wine and not worry about waking the kids. We could also use the kitchen and washing machine.

Also, we like our food but obviously we couldn't go out late in the evenings because of the kids. So, our hotel would pick up food from restaurants. This wasn't limited to normal places either. We had Me Chows one night who I would never expect to deliver.

So, that's my advice, look for a hotel that has residences in it. Best of both worlds.

Might not fit all of your criteria but the Grand Hotel in Costa Adeje, Tenerife may be worth a look. My wife and I hope to go in November.

We stayed at the hotel hipocampo palace hotel in cala millor in Majorca this summer with a 15 month old.

I think it meets every single one of your requirements with the addition of an indoor pool if needed.

The breakfast was superb, and the beach is sandy and the water shallow. Kids pools has loads of free toys and there is a play area next to the pool.

Room wise, we were upgraded to a suite which gave us the second room and the hotel provided a brand new cot and gave the little one a bunch of freebies.

Resort restaraunts are maybe the only thing that wasn't superb - that said there were plenty and a few were pretty good.

Little kids were well accepted everywhere with high chairs etc always easily available - made for an easy going holiday. Would definitely recommend the hotel.

Have done the following with small people over the years at end of September / beginning of October:
Sani Village - easy direct BA flights to Thessaloniki and hotel will collect you with car seat
Forte Village - lots to do although hotel did have more conference guests than in May - sea however much warmer
Almyra in Cyprus, very geared up for smaller children - bottle warmers etc, sea view or suite essential as standard rooms face the road, not much beach though
Bahia del Duque - Tenerife, lovely setting, nice beach in front of hotel - Spanish beaches aren't private though
Daios Cove, Crete - have done a few May half terms here recently, we haven't been in early October so you should check the weather but its a lovely hotel with beach and fantastic facilities.
