Terminal 5 Heathrow

I'm looking at planning my European trip this year, and I did ask a question about Geneva, but due to changes in flight from Asia it now looks like I will be flying
Kuala lumpur - Heathrow -Milan, with BA, flights are all Terminal 5, does any one know if I need to go trough passport control to make this connection, I will be travelling with an American friend, my wife who is a UK resident but not a citizen and my friend's wife who need a visa for UK, There are two problems (1) need to get visa, and (2) if you can not take the EU line in passport it is very hard to judge the time it can take.
