Broaching the subject of her weight gain

You could ask why she hasn't worn XYZ in a while (a nice dress or something that you know no longer fits her).
If she replies that it doesn't fit her any more - reply with "pity - I really loved you in that dress".
By making her draw attention to or acknowledging her weight gain - you will likely avoid a slap.

How long did the weight gain take? Is this the work of a few years or something that's changed quickly? Either way, she knows about it, you can be sure about that.
If its happened quickly, you can just ask, is there a problem because you've put on a good few pounds lately. She might be waiting for you to say something.
If its taken years, why are you just worrying about it now? Maybe you could try, remember when we used to be young and slim? (whether you still are or not). Depending how she responds, you should be able to pick up her lead
If she says, well those days are gone forever, you can say, they don't have to be. If she says she'd love to be slim again, say you'd love to help, diet and work out with her. Even if you don't need to - to show that you mean it.
