I'm struggling to understand why sleepiness would be such a biggie if you were terminally ill.

Well nowadays it is strictly limited but back in the olden days it was used quite liberally for tooth ache etc. Also someone mentioned the use of cocaine for eye surgery but normally you do need the iris to relax so atropine would also need to be administered i think.

0·5% tropicamide drops or

0.5% cyclopentolate

Or atropine. tropicamide requires another drug to be use with it i think, phenylepherine?
