the biggest problem is that amount of weightloss can be deadly. These restriction diets are not healhy in the long run.

LighterLife’s VLCD and its counselling component may work for some, particularly people who have struggled to lose weight for years, have health problems as a result of their weight and are clinically obese with a BMI of more than 30. A VLCD that involves eating 1,000 calories a day or fewer should not be followed for more than 12 continuous weeks. If you are eating fewer than 600 calories a day, you should have medical supervision.'

The problem is that 500 calories a day is about a quarter of what's needed for a man. So at some point he's going to need to ramp up the calories significantly, and that's where the issues could occur.
I have to say congratulations to him though, he's lost the equivalent of giving me a piggyback 24 hours a day.

He'd have lost more if he didn't eat a thing for 6 months, though... now, given that my body needs 1600 just to exist (for brain activity, keeping organs doing their things, bowels moving), what happens if I eat 500 a day (or indeed nothing) for 6 months? What really happens? Any docs care to answer?

Im not a Doctor but id presume you would end up looking like Tom Hanks at the back end of Philadelphia.Hurrah! Man loses 11 stone, 12 million brain cells, takes 15 years off his heart, pancreas and kidneys and he didn't have to do a day's exercise! All celebrate the total victory of laziness! Go eat, consume, spend because nomatter your lard storage, you can clear it like st out of your toilet by something as easy as pulling a handle...
