Hasn't your wife answered this for you?
No, she has also never used a Doula so doesn't know what type of service they'll provide.
But you said above "Wife isn't bothered - she says if it'll make me feel better to have someone else there then go ahead, but she also doesn't want someone bothering her with spiritual mumbo-jumbo".

If she's not bothered why are you? she's the one giving birth, i understand there is apprehension for the first child but i think you should go with what she says.
This is my answer too...

if you enjoy blood/gore then you'll have a good time laugh

The NCT classes are useful for the information but what is of greater benefit is having a group of friends at the end of it who are all in the same boat. Over 5 years on and we're still in contact with all of them even to the extent that some of them are coming over to ours for a BBQ this weekend.
Oh yeah, it looked like the scene from Alien when ours was born, and she came out the "normal" way. Actually, she was dragged out with a sucker on her head, but through the correct passage...

I was keen to stay stay at the head end and managed for the whole thing, til they gave me the baby to hold. Had to walk round to get it, and got an eyeful! biggrin

As for classes, we just want for the standard free ones. They were OK, our midwife did one week and she was down to earth and a good laugh (and prepared us for said sucker), but the other weeks were a bit dull. How to put on a nappy and the like. The other people there were all a bit quiet or odd, so we didn't really make friends with any of them. However, my wife is friends with quite a few new mums in the village so that's all good.

First was an emergency C.
I made the mistake of looking around the "curtain"..

It looked like something from a Ray Mears show.
