"The hookworm larvae will burrow into the skin of a human’s foot or leg causing a linear, red lesion, which is intensely itchy. This parasite has also been known to cause chronic intestinal bleeding, abdominal pain and diarrhea in small children. Deworming medications usually include pyrantel pamoate and fenbendazole."

Ok as Dave is rather tied up at the moment, I'll try to give a brief answer to some of the concerns.
If your toe is itchy, it is likely that you have have a fungal infection, which can affect the nail as well as the skin, and will make the nails look like the ones in the picture obtained from the internet.
You can treat it topically, by painting on a medicated nail laquer, prescribed by your GP, or you can take oral antifungal tablets, but 3 weeks is nowhere near long enough for nails, you need 3 months worth, and then up to a year for the nails to grow through.
The guy with the red line - that looks fungal too, you can use a topical antifungal cream, ask the pharmacist to recommend, or the GP to prescribe.
Right. Ingrowing nails. 2 methods of removal, both can be carried out under local analgesia, not pleasant, but should be similar in discomfort to dental LA.
Method 1. Phenolisation - section of nail is removed and phenol applied to the nail bed and matrix (growing part) to prevent regrowth. Around a 95% success rate. Takes about 6-8 weeks to heal as the body has to deal with the phenol.
Method 2. Incisional, matrix is cut out and the toe is stitched up, more painful initially post-op, but heals quicker as it is a surgical wound rather than a chemical burn.
Please do not assume that private is better than NHS. Our Nail Surgery service is extremely good, we are very proud of our success rate and we get many letters and cards to thank us for our treatment - sometimes after people have had bad experiences with other people in the past.
Hope I've covered most points!

Similiar in discomfort?! You must have a maniac for a dentist!! I've had the LA in toes and it was agony - sheer undiluted agony!! But it's not much better in feet anywhere as I discovered to my cost when having a wire removed post bunion op - neeedle went in foot - I fainted - but was on bed thing in BUPA hospital and cardiac failure button was directly behind me ... I fainted with the pain, hit said button and came round to cardiac team leaving room looking like that cartoon of vultures disappointed at no kill!! Oh yes - the surgeon was still at the foot of the bed totally oblivious to goings on and digging away happily at wire...

Brilliant!- sadly they just seem to be taking a very long time to take effect for me, the Curanail kit is slowly working and certainly tided the main offender up a bit.
Its the itch thats worse though - I can't work out what triggers it and it just seems to move around that area of my foot. Luckily I work from home most days so I dont have to wear socks at my desk.
Two other questions:
1: Why do we have nails in the first place? They just seem to give 45% of Pistonheads trouble.
2: What on earth causes bad nails to create that terrible smell? Is it literally rotting away under there?
