I had this pain a couple of years ago. Due to being a prop forward, I have damaged vertebrae (7 & 9 iirc), and this caused problems with the nerve that runs from the neck, across the shoulder and down the arms. As you say, the pain moves about. Anti inflamatories didnt work. Luckily have private medical, so after several visits to the specialist, including injections and scans, it seems the nerve/muscle was inflamed (cant remember the medical term for it) where it passes through the shoulder joint. At a certain age (40s) this hole in the shoulder can calcify over, so inflamed nerve and smaller hole meant the nerve was continually being 'pinched' (impinged?).

In the end I had keyhole surgery where they basically scrape the calcium away, making the hole bigger again.

The pain is mostly gone, although I do have the off flare up after a particularly hard game bit of scrummaging, but it goes after a day.
