You don’t usually have to pay VAT if you’re importing a second-hand vehicle and the VAT was paid in another EU country.
You’ll have to pay VAT if your vehicle is classed as a ‘new means of transport’ - ie it’s less than 6 months old and has done less than 6,000km (about 3,728 miles). HMRC will tell you if this is the case.
ETA: Out of interest, what have you found that's cheaper there than here? Most cars are stupidly expensive in Malta.

Malta's EU. So usual EU answers apply - CoC if <10yo, just MOT if over. No tax, no duty, need HMRC NOVA cert. V55/5. MOT & insure on old plates or VIN.Maybe go on the Classic Car forums.

3 days+ to drive back, not sure I would be brave enough in a classic Fiat, though it would be an epic drive. 2 days in Malta (nice country, lovely people and outside of high summer, lovely climate), Up through Italy, across into Switzerland, then up through France. Good motels and some restaurant hunting, would make a holiday.

They do go a bit doolally in the rain, mainly because none of them pay any attention during their driving lessons and nobody knows any other route to their destination than the one they always use, so the main roads get jammed and are often rivers...I know lots of MK1/2 Escorts got imported once the prices over here got silly.

Lots of old cars survive because they are only driven by their original owners on high days and holidays.
