My car is up on 4 axle stands...

If I am working under the car, place 2x4 timber or more wheels on top of the wheels under car. That way, if it falls off the stands, it can only drop a few inches, and there is still plenty of clearance under the body.
If you work around cars long enough, you will see one fall off stands or a jack.

It depends on the height, but I use two trolly jacks per end to get the frong wheels onto my ramps, before lifting the rear the same way and working around the car two big bottle lacks and stands/wooden boards until its really high.
I have also modified my 4x stands to spread the load over a greater area of chassis rails and I also use the two bottle jacks as a 5th/6th load point.
Bottle jacks are good because the only go upward, but you need to have the height in the first place to put them under the car.
