You are getting the slip when you are running at higher torque levels so the clutch is not clamping fully. I suspect that the problem is not driver related, but down to the release mechanism sticking but possibly other issues compounding it. That could be at the slave cylinder, but there could also be a pedal return spring problem. Check every single part and make sure none are missing. This is often the case when pedal return springs break and they can leave a clutch looking much like yours.

Oil leakage contamination possibly,look at the inside of those fingers frown they certainly look damp with something.
