Rear exhaust hangers

Although you don't have to removed the rear diffuser, it does make life easier if you do.
There are two bolts in the boot for the exhaust heat shield that need to be removed, and then the bolts under the rear diffuser. if you don't want to remove the diffuser, just undo the other two bolts on the underside to remove the heat shield.
It is advisable to use a smidge of copper grease when screwing the bolds back, so there's less chance of them seizing - especially for the rear diffuser itself. I also changed all of mine to stainless steel while the diffuser was off.
ETA - Just realised you have a T350, doh!

For the T350 you need to remove the diffusor to get to the exhaust hangers. No need to jack the car up necessarily, although everything should be easier to reach.
Also depends on which hanger you mean: The one at the main frame holding both exhaust pipes before the rear can(s) or the two hangers where the rear can(s) are attached below the boot area.
