Recently I have been training a lot. I monitor my resting heart rate and HRV daily. After my last event (half ironman) I rested for a week and then got back on it. I have noticed a sharpish drop in my RHR and a few times when getting up out of a chair I feel a bit dizzy.

I have swapped sensors now and do the measurement sitting down rather than standing but my average RHR has dropped from 67 to 54 in a few weeks. This is taken after having a piss so not completely at rest.

BP is ok at 121/72.

I am a big person as well 6'5'' if that makes any difference. I have had some performance blood markers tested and the results i have so far (Iron/Ferritin) are fine.

Is this just because the fitness ahs lowered my RHR or should it be checked out?

I'd say take no chances, if you are feeling dizzy, get it checked out. Your RHR sounds low to me, but I'm far from an expert which is what you need, to put your mind at ease if nothing else.

How old are you? Any medical history? Any family history (did your Dad have heart problems etc)? Ever had an ECG?

I'm the same. Resting heart rate in the high 40's and if I'm not well rested after exercise I find myself holding on to stuff to stop myself falling down if I stand up too quickly. It's worse if I've just been out exercising in the heat or if I come in and have a hot bath straight away.

I've never been fitter and my blood pressure and cholesterol are bang on. I guess it's just a byproduct of the low resting heart rate.

Used to happen to me a fair bit when my RHR was in the forties. Now I'm less fit it doesn't happen frown

36, fit as a fiddle (well a little over weight now but have lost a chunk and losing more). No medical history of anything. I had an ECG done about 4 years ago when my blood pressure was a little high (caused by too much booze and being overweight) and that showed nothing. No heart problems in the family at all.

I don't want to worry you unnecessarily, but definitely get it checked. Around last Christmas, my next door neighbour was suffering headaches and dizzy spells. He did go to the GP, but as of last weekend, he's no longer with us.... frown

the sudden drop in the rhr may be something. but without a physical examination, ecg and postural BPs it;s going to be hard to tell ...

GP ASAP I think ...

Can I suggest am ECG that lasts 12 seconds isnt going to show a lot ,id ask for an echocardiogram if I were you or at least plant that seed in in your GPs mind. doesnt hurt just lie there while they run an ultrasound looking piece of kit around your heart good luck

Definitely get it checked.

Odds are, it is fitness, think they call it athletic bradycardia or something, especially in combination with you being a tall guy, postural hypotension and all, but you CANNOT take the risk of assuming it's that and everything's gravy, it needs to be sussed out properly.

Once issued with a clean bill of health you can then bask in the smugness of being so fit that your heart is strong enough it doesn't even need to beat once a second to do the job properly… well, mostly properly. In the meantime remember to move slowly when standing up cos even if it is just athletic thingywhatsit it won't help a blind bit if you black out and brain yourself on something on the way down!

and your CCT in either cardiology or radiology was gained when exactly ?

It would be considered 'amateur sleuthing' and against PH's conduct guidelines to check the GMC register looking for cardiology or radiology CCT holders of similar names to that given in your profile and post up whether or not there are any ...

Actually, a resting ECG can tell you quite a lot. Given his symptoms, I doubt an echocardiogram is really necessary, unless the physical examination throws up a likely structural problem.

Actually, a resting ECG can tell you quite a lot. Given his symptoms, I doubt an echocardiogram is really necessary, unless the physical examination throws up a likely structural problem.
Hence my earlier remark, in several years of A+E / CDU /AAU practice I am unaware of anyone being sent for a echo for postural hypotension, ditto with a number of years working in spinal / neuro rehab.

plenty of resting ecgs, postural BPs / tilt table and so on ...
