If choosing a hosting solution for a website, at what level of traffic/DB hits and so on would a dedicated server become necessary? What are the likely costs involved?

It entirely depends on what you're actually serving (dynamic content, video content, database driven content, forums etc)

Static content and large downloads aren't really that CPU intensive, so you can service a lot more clients than say a Forum, or blog / image gallery / something 'web 2.0'

As to when you need to make the switch, this is partially down to budget (ie if you have a pseudo decent budget, get one from the start for the additional flexibility, security and resources it affords) and the traffic content and profile mentioned above.
