Looking for some advice on a care company in Guildford.

Wife's Grandfather is leaving hospital tomorrow, he has been in Hospital due to issues with blood platelets.

We need a care company who know what they are doing with palliative disorders

Any help greatly appreciated

Local council?

I'm not in your area, but mine has a palliative team.

Speak to hospital discharge team/person.

It gives you a list of home care agencies in the Surrey area (got it from the link from the council website (http://new.surreycc.gov.uk/social-care-and-health/care-and-support-for-adults/care-and-support-where-to-start))

The document may well give you other useful information so it would be worth looking through it.

I should have add that if the hospital think your wife's grandfather needs social care support on discharge they should have sent a referral through to social services and their hospital social work team should be assessing to see what support, if any, he needs. Social services departments usually have an intermediate care team, providing free care and support for up to 6 weeks. The hospital really shouldn't be discharging the grandfather if he cannot cope at home on his own.
