Why does PH insist on no naming and shaming? It would actually be very useful for users to hear firsthand from other members when they have experienced great service or otherwise. A bit like an automotive Trip Advisor...
Because it might affect their advertising income. Not going back through the thread now but Google the chief execs name and email address and get your complaint Ito him. Also bill them for your time, say you feel that half an hour is reasonable but over this is unacceptable and my time is billed at X per hour. You probably won't get it but it might make them sit up a bit.
Insurers don't get it right on many occasions ( I'm an ex employee) I've had pro Rasta refunds horrendously miscalculated in their favour. Just yesterday I had a settlement on a claim for my mum it should have been for £895 yet they've sent £1195. That's going to be fun sorting out!

This may sound over the top but MCOL is what you need.
If all you've said on here is the way things have transpired then just write a short letter to HQ giving 14 days notice to get a full refund and state you'll begin proceedings without any further reference to them should they fail to issue a full refund. Make sure yoi send by recorded delivery.
If they dont refund you, issue a claim online (£15), sit back and await your full refund.
If this doesnt work, I'll pay your claim fee.
