Clutches may fail, and slip, and do many nasty things.
But for a clutch to get to that state, it is 100% down to the driver.
Yes, they clutch may have started slipping for other reasons, but to end up as bad as that, the driver must have continued abusing the car, or driving hard with the clutch slipping.

Note there was absolutely no clutch slip what so ever just changed gear from 5 to 4 and it wouldn't engage, as for driver error , I don't do burnouts,180's etc and am not an opposite lock monkey so hardly driver error!! the original owner of the car doesn't come across as this type either .Only thing I do is change from 2nd to fourth or 3 to 5 during acceleration. Also some times on slowing 5 to 3 or 6 to 4 to 2, have been told this doesn't help so have changed driving to suite.

Similar look to mine when it went. Spun centre on the friction plate and split from the inside of the friction material out. A right mess which I'm sure Test123 can vouch for. I don't do burnouts or flog it to within an inch of its life either. Previous owner however wasn't the most gentle with either the left or right peddle.

Still here, never abused the car, never even made the back end slide, strut brace and rip shifter were cheap and effective mods, ( could not tell the difference the strut brace made but it looked good ).
Only back 2 wheels were curbed ( turning into a shop entrance and crawling speed ), never had issues with brakes as they were never heavily used.
