Quick Ltd company formation - recommendations?

I have registered my own new Ltd Co at Companies House before, but tried it the other day and it got sent back! I had not completed it correctly. Need one pdq and have run out of time. Any recommendations or guidance. I need it by end Monday.

There are lots of cheap and effective formation agents out there - most are now on the web. Two of the best known are Stanley Davies and Jordans. Both have been in business many years and any of my clients who have used either seem to have had no issues.
