Contractor terms of service - template?

The other half has been working on a temp contract for a company - the contract is up and they have asked her to stay on.

Rather than become employed by this company she will be operating under her own business name and invoicing them for the work done.

Does anyone know of a suitable terms of service template that could be used in this instance?
