Flaking paint in the bathroom

I really need to redecorate the bathroom, the existing paint is peeling off the walls in several places, presumably due to the humidity, what is the best course of action? Do I need to rub it all back before I paint (I have tried and the paint continually lifts around the edges which havent been sanded).
