Vets/opinions needed please for a Westie skin issue

Hello!

Our In laws, have a Westie with some skin complaints. It doesn't seem to bother her, sometimes she scratches but not often.

I've added so pics, hope you have all had tea.

She has had this for about a year and the vet prescribes anti biotics which do work but doesn't last.

She's tried the raw diet which didn't help and currently has higher quality dog food.
