Sounds quite like the longer term results of diabetic nerve damage. Numbness and neuropathic pain go with that particular territory. Your local pharmacy ought to be able to offer you a thumb prick test for sugar levels whilst you wait. You might then want to get an HBA1C blood test which will g8ve your sugar readings over 3 months.

Try placing a bedsheet over the toe. If the pain is excrutiating, its probably gout.
If you can walk with it, its probably not.
IMO either your pain threshold is high and you don't make a fuss over things, or it's something else. Gout is probably the single worst pain i've ever felt and that includes broken bones.

I thought gout was more out and out pain rather than numbness and tingling.
I get numbness in one of my feet/big toes but it's related to sciatic nerve issues caused by a herniated disc about 10 years ago which in turn was brought about by posture changes due to a severe ankle injury in 2001.

Well yeah, I take after my father, I'm not sure whether it's a high pain threshold or idiocy. One of his mates mangled his finger by accident, I took him to hospital and he was asking if the Dr could quickly cut it off and sow it up so he could get back in time to feed the sheep. If it was peripheral neuropathy from a systemic conditions it's usually always bilateral and not just isolated to a single toe.
It could be gout, pseudo gout, secondary to OA changes, medication side effect, vitamin deficiency anaemia, biomechanical origin, neuritis (Joplin's neuroma), gunshot wound, lumbar radiculopathy, space occupying lesion, lower motor nueron lesion, sesamoiditis etc etc etc.
Won't hurt to go see your GP and see if some baseline bloods are in order and then ask if you can access an msk specialist podiatrist.

I have been through various degrees of diabetic peripheral neuropathy in my hands and feet. Nearly fully recovered now, thankfully, but I want to add that for me, it was almost exclusively on my right side, so definitely not always universally bilateral. It started out only as numbness, but turned to pain as my body began repairing itself and nerve function started to return.

Not saying it's gout, but beer and (red) meat are quite high up on what will upset your uric acid equilibrium. If I'm heading into a flare-up the warning signs (usually in my left big toe) are tightness of the joint (at the knuckle) and numbness. There can be some tingling but the onslaught of pain is like having the ball of your foot clamped in a bench vice and then someone putting the kind of pressure you would need to press a ball bearing out of a hub on the knuckle.
