Refund link for credit note

Is it me or does the refund form make no sense at all?
It's asking me 'Nephew' in french then 'Frederick' in the next bit then random pictures and numbers, all with a star to say it has to be filled in.
It's just bonkers!
Tried opening it in different browsers but always nuts!
Anyone want to explain it to me. Cheers
