Fuel cuts off at 3200 revs - Totota Avensis

1998 Toyota Avensis 1.8 GS.
The fuel is cut off from the injection system when the engine reaches 3,200 revs. Occasionally it rises to 4,200 revs. Below those revs it is fine, except occasionally it is a little difficult to start.

Any ideas?
