Not sure if anyone has brought this up in this thread but have you heard of the 5 stroke being developed?
it uses 3 cylinders, the two outer cylinders exhaust into the middle cylinder which is low pressure vus using the burnt gases to push the piston down.
I guess similar to a compound steam engine.

I think one of the best engine designs I ever came across was one that could be started with one finger, It was self-lubricating, accepted any size piston and even changed its own oil every four weeks.
Management system was a nightmare though.
