USA W9 Form Help Please

Is this a Non-Resident Withholding Tax form?
If so, your US customer will be obliged to retain a percentage of any payments to you unless you complete the form, and have your status as a UK taxpayer verified by HMRC before sending the form to your customer as their proof that they are paying an unconnected non-US beneficiary.
You can find guidelines for completion of the form online, via HMRC. It won't be the most pleasant way that you've whiled away an hour or two, but if you want to receive payment in full, you'll have to complete it, and make sure that you keep a copy for your own records.

W9s are for US entities. If you are a UK entity, you need to complete a W8. Your client sent you a W9 because most US companies have US suppliers, so they are accustomed to W9s. And there are two versions of W8, one for companies, one for individuals.
The form is not specific to a company, there is only one form nationwide, so you can download the W8 yourself from the IRS website, fill it in and send it to your customer.
