Format for affidavit

What's the format for an affidavit? I have a land dispute and want someone to write a supporting testimony. They will write about 300 words, there will be one figure (a map) and that's it. As it stands page 1 has the text, followed by the signature locations (two of them), and then the map is on page 2 under the title: Appendix A (presumably if I keep this format I need to add an additional signing location on the map page).
But can I arrange things so that I put the map in the text (so it's not on a separate page but embedded in the text), and then have the signature at the end. And I can then put a caption on the map, eg Figure 1, and then refer to that in the text?

Affidavits are not generally used in court proceedings these days, and evidence is normally provided in the form of a witness statement.There's some good guidance here - https://england.shelter.org.uk/professional_resour...
