On a two lane carriageway the right hand lane is no longer of overtaking, and is now for use by anyone who 'might' be turning right at anytime in the next 20 miles.
I'm convinced folk do it who aren't confident enough to get into the right hand lane when they actually need to, so instead they sit in that lane all the time.

I had the fright of my life while in my car with my young daughter thanks to one of these idiots.
I joined the M20 at 70mph behind a large truck which was in the driving lane of the M20 and went to overtake only to be confronted by a car in the middle lane hidden by the lorry, doing around 40mph in the centre lane. The truck was undertaking the slow car which I was not aware of and fortunately I managed to slow down in time.
Earlier this year I saw a number of unmarked police cars pulling over a number of drivers near the A3 slip road on the M25. As was stuck in heavy traffic I would down the window to hear an officer ask a driver why he wasn’t keeping up with traffic in front and why he was blocking the overtaking lane by not overtaking. Need more of this as is as serious as speeding

Can we also sort out those drivers, like the 4 car/pick up drivers who joined the M1 yesterday at junction 25 going North, instead of using the left hand lane of the slip road (longer time to build up speed, gauge gap to join, better visibility etc than the right hand lane) used the right hand lane (shorter, lesser visibility) to get onto the main carriageway? Literally all moved over at the top of the slip to give themselves much less time etc to join the motorway, it doesn't make any sense to me whatsoever.
The mentality is literally 'get onto the motorway to sit in the wrong lane a as quickly as possible, and to hell with safety'.
It's a scary time we're living in.

Similarly, the bus lane on the stretch of the A406 through Palmers Green is used by people who 'might be' stopping to see someone in one of the houses. I agree this is a real issue and impacts safety and the flow of the motorway.
But, on the “smart” motorways at least, wouldn’t the camera coverage mean the government’s super computer can detect lane hogging easily / automatically and send “words of advice” letters to the drivers? Followed by a fine for a 2nd offence?

Although if the culprit is driving at 70mph, he can't be causing congestion can he... and is preventing other drivers from breaking the law... which I suspect is exactly what some of them are thinking.
I'd suggest roadworks which go on for months with no sign of work, and the associated 50mph limits, now sometimes 40, are far worse culprits for congestion. Sort those out.

Obviously the 40mph MLM was wrong, however I'd say you are 90% responsible for your brown gusset there. Fallen objects on the road are not uncommon, and stranded vehicles on the carriageway do happen. I see probably one per year and I only do about 10k miles. You'd have spanked into either of those hazards.The reason you are 90% to blame is that you shouldn't have been moving out without sufficient view of the lane you were heading into. More distance between you and the truck in front would have given you longer sight lines and moving out towards the lane without actually swooping into it would have allowed you to see round the lorry without putting you on a collision course with the MLM snail.
TLDR: never change lanes without having sufficient view of the lane you are moving into.

A similar type of slip road, where the two lanes of the slip road merge and become a new lane in the motorway, are infested by people who don't read/understand the signage and move into what is now the middle lane despite already being in a perfectly good, empty lane. Not enough information. Maybe as someone up there ^ said he was already doing the speed limit so how can he be holding up the biker.
Secondly perhaps the driver deliberately stayed out because the biker was being a knob, which wouldn’t surprise me. On that score we will never know / be told.
