Threshold for accidents and 'driver improvement' courses?

Many of us have attended Speed Awareness Courses having been caught exceeding a posted limit.
Some may feel hard done by as their infraction didn't have any victims and endanger anybody.
Should avoidable and foreseeable road traffic 'accidents' that result in victims also be dealt with by means of paying to attend a lecture reminding us of driving standards and possible things that can go wrong?
Experienced and learned posters in SP&L freely and helpfully give advice on penalty guidance for speeding and other driving offences.
Is there a threshold that determines whether a driver responsible for an accident is dealt with via a 'driver improvement' course instead of prosecution? Or is it determined on a 'case by case' basis?
