Changing child surname with Statutory Declaration

Quick question which I hope someone can help with. Struggling to find out by Googling.
We're renewing my son's expired UK passport. The complication is that his surname has changed from Smith to Smith-Jones since the last one. The passport office wanted us to provide a statutory declaration of change of name witnessed by a solicitor, which we did, but as he's under 16 we should have signed it too, so they rejected it.
So my question is, can we just use the same letter template and add a space at the bottom for us parents to sign? Or should I re-write the letter so that it reads "we" and doesn't read as "I" any more, as it's not written by him?
I've tried the passport advice line and they are not very helpful with specific advice.
