DVLA over 70 renewal fudge up

We found out today, quite by chance, that my dad had been driving on an expired licence for the past 2 and a bit years...
Looking into it, it appears that when he renewed his licence on turning 70, back in 2019, the new licence he received had the exact same expiration date, rather than having been extended by 3 years as expected. He followed the guidance he was provided, did it all online and received a new licence, issue dated 1 day after the renewal application, just with no extension (so expiring approx 3 months after the issue). He received no further reminders or notes, luckily he keeps literally everything so we are pretty sure on this.
Obviously he is now a bit miffed, since he has to wait for a new renewal to come through, hopefully without DVLA manual intervention given the mess they have right now. Worse, since the expiration date was over 2 years ago -June 19 - he may face a retest? If that happens he really is up s
t creek time wise.
Any experience from anyone of anything similar? Obvs we will pick it up with DVLA if we can manage to speak to anyone, but what to expect would be good (apart from not much help from DVLA, given past experiences)?
Any idea, apart from a bungle, why his original licence may not have been extended? There are no medical issues etc, points, DQ or anything - it should have been really simple (hence his assumption and not checking it when it came back).

) but practical advice much appreciated.Go on the DVLA website and check the licence. You will need the licence number and national insurance number. Then take it from there, renew if necessary (it may be a misprint on the plastic card).
Note: I had to surrender my licence in 2007 due to epilepsy. Back then it was a three year wait until I could renew (1 year wait now). No retest needed then. I very much doubt a retest would be needed either in his case.
