Blackout in car - reapplying for license

At the end of July, I had a blackout in a work car and ended up in the hedge, following months of unexplained (and unsolved by docs) headaches.
At the time, the A&E doctor said she was concerned I had had a fit. Having then read it was better to send my license back rather than having it revoked, I did so in the days following.
I was referred to a neurologist who said he thought it was most likely down to migraines and not a fit, and said he wouldn't have had me surrender my license 🤦♂️
As I had already done so, he had me wait until a CT, MRI and EEG had been done before reapplying. These have all come back clear and so he's put it down to migraines, which are now controlled on medication.
As far as I understand, I reapply using form D1, then am able to drive under Section 88 once the application is posted through the DVLA's letterbox? Or do I need to wait for potentially 6+ months for the DVLA to process it?
On form D1, there is nothing about a surrendered license, only revoked ones. I assume I am just meant to mark that I'm reapplying for medical reasons, and then blackout in section 4. I just don't want to shoot myself in the foot filling this in, if it's going to come back to bite me in 6 months. But that seems the most correct way to fill it in.
Regarding Section 88 - if this does apply, how does one prove that they're eligible under it if ever stopped / questioned?

Seriously?After going into a hedge? The first doctor was correct. You went (presumably LEFT into a hedge). What if you had gone RIGHT straight head-on into another car?

The blackout occurred with a particularly severe headache (it had not done so with bad headaches before) - his advice was don't drive if I have a bad headache, in case it was a trigger, but he saw no medical reason affecting my competency to drive.
You don't think that thought has occurred to me many, many times.

Once lost about 90% of my vision when commuting home on the M1. Vision covered in jagged lightning like lines, Got from fast lane into a service station, luckily.
After 4 or 5 of them I could tell if about to come on - and a couple of times had to park up and sleep in the car for an hour or two.
Obviously off to the docs with this story: cutting a long story short and it turns out that the constantly refilled filled cup of coffee on my desk was to blame. Cut out all caffeine and they ended, just like that.
It was never suggested that I stop driving but I did use the train to commute for a few weeks as we went through a food elimination process.
Caffeine, chocolate and oranges are common triggers.

I know of someone who went to the docs for a routine check up and he mentioned he had passed out on a flight the year before.
The doctor immediately informed the DVLA that he had revoked his licence for six months and oreder he had a scan.
Scan came back all clear, however by the time he went through the hassle of trying to get his licence back sooner, it took the remainder of the six months to get it back and said he may have well just waited it out.
