Looks like the ground was too soft, The area looks newly concreted so the area around it might have been dug up causing it to become weak then the added weight of the counterweight passing over the stabilizer leg took it through the surface.

As an Appointed Person, this is the sort of thing that gives me nightmares! Crane was only a few months old, a '14 plate I believe. I've worked with these 500-tonne, and 750-tonne, cranes in the past and the weights involved are incredible - probably around 140T of counterweight on the back of that, plus the 96T weight of the crane itself. A miscalculation in the spreader sizes, or of the ground conditions may have caused it.
