[Gardening] Help me grow a lawn!

There's a small piece of land I'm trying to grow a lawn on. I reseeded it last year in August but to no avail really. I just went to have a look and it's in a bit of a state at the moment, really compact sort of mud and quite a bit of moss. The other thing I noticed when I raked it (clearing leaves) the roots tore out really easily.
What should I be doing and when to get a nice lawn? Spike it? Rake it? Kill the moss?

In slightly more seriousness, anywhere I have had a lawn to look after I have done my best to reduce it in size.
I am no expert, but a decent lawn always requires lots of hard work.
I am tempted to say just keep plugging away at it, but it needs drainage and water, food and aeration and (as soon as it's grown strong enough) regular mowing.

How bigs is the lawn?
If it is not too big then you need to go over the entire thing with a fork and stick the fork in about 6-8 inches, opening up all the holes. You then need to sprinkle lawn sand all over the place (ideally this should be done in the early autumn, but it will still be beneficial), and brush the lawn sand into the holes. The holes will aerate the soil and the sand will assist in drainage.
The really bare patches need to be raked over with a spring rake and you need to mix some grass seed into a bit of potting compost and spread it over the scruffy patches in late March or early April. If you have a bird (feathered) problem then spread some netting over the top, supported by sticks, until it has germinated, usually a fortnight.
Cut it only gently for the first few cuts in April/May/whenever you get to mow it, don't sheer it off to ground level.
Weed and feed in May and water when dry.
Or buy some astroturf as it looks prettier than concrete.
