I Need A Strategic Plan Template

Hi can anyone help me out here...Have been asked by my CEO to present a 12 month Strategic Plan. Have'nt had to do one in a while so im looking for any help or pointers as I have had no joy finding any suitable templates or examples...all help or links most appreciated.

Strategic plan for what?

I have done this regularly for a number of business and from a number of angles - Op's, HR, IT etc.

Normal format is:

1. 12 month look back with highlights and things that haven't gone so well.

2. SWOT analysis

3. Objectives for next 12 months - linked to overall corporate strategy, with key challenges

4. Resource analysis and budget

Simple!
