Insurance claim advice

Bit of a strange one, so my girlfriend has been looking for a new insurance quote and when getting a quote she has been told she has a claim (she’s not aware of this at all). The claim was on the exact same date she was in a crash as a passenger (not in her vehicle - in a completely separate vehicle that was written off by the crash she was a passenger in)
The insurance company she has tried to get a quote from has stated that supposedly she was the driver and it was a 3rd party claim made against her. The crash didn’t involve any other vehicles at all, was simply just stupid driving and crashed, rolled and written off.
Has anyone got any idea how this could possibly be put in her name or related to her at all? Or any advice where to go from here / how this has even happened?
