Is this a coil/s injector/s

Maybe an Italian style tune up (rag the bejeezus out of it) and maybe a dose of injector cleaner etc? Have you removed the plugs? Dropped any oil from the sump?
Check for metal flakes in oil filter or sump oil.
Is the fuel pump/filter old? Could be starved....?
Also the smoke in the engine bay - has the inlet manifold cracked?

It's my wife's and seemingly just started doing it. I haven't looked at it yet. It's stuck at her work place in going to see it later but hearing the short video sounds hellish. Check for metal flakes in oil filter or sump oil.

No there's nothing been done that you mentioned and anything that can be done will need to be done at the roadside. I've never heard a petrol engine so rough.

In the absence of any diagnostic equipment I'd start by unplugging all 4 injectors and cranking it on the starter. You can tell straight away if your problem is a mechanical failure (no compression) or a fuel/spark issue.
Once (if) you've ruled out compression you can then disable one cylinder at a time with the engine running and listen for the one (or two) that makes no difference. You can then swaptronics your way to an answer in most cases.
In reality I wouldn't go about it like that at all as a £10 code reader will allow you to speed up the whole process no end.

I would start by finding where this is coming from.You can't possibly diagnose a problem with bigger one staring you in the face, especially when it could very easily be part of the issue.
I know but a loose or cracked manifold won't cause a bad misfire and cut out. Well not that I've known as I've did it through the years with other engines.
Checked for codes with a noddy reader i have and nothing came up for anything which is odd. Unless the bugger had them cleared which wouldn't surprise me.
